Welcome to your CGM Support Service
Now you’ve collected your CareSens Air Continuous Glucose Monitor (CGM), it’s time to begin your 12-week support programme.
Over the next 12 weeks, our specialist diabetes team will review your readings each week and get in touch if anything needs adjusting, helping you understand your numbers and feel more confident managing your diabetes.
Our Spirit Health team will support you throughout this time, working closely with your GP practice to make sure your care remains joined up.
How the service works
- You wear your CareSens Air CGM as normal
- You connect your device to our healthcare team in the CareSens Air app using code 329912 (not sure how to connect? Watch this video)
- Your glucose data uploads automatically
- Our healthcare team reviews your data every week
- If advice or changes are needed, we’ll get in touch
Your CGM supplies will continue to be provided through your GP prescription as normal.
Most weeks, you won’t need to do anything. If you don’t hear from us, it usually means your readings are stable and on track.
How to get the most from your CGM support
To help us support you properly:
Wear and connect your CGM
Wear your sensor every day and make sure you’re linked to our team in the app using code 329912.
Keep us in the loop with your health
Let us know if you’ve been unwell, more active than usual, or under extra stress. These can all affect your glucose levels.
Speak to us before changing insulin
Please don’t adjust your insulin doses on your own. We’ll review your data weekly and guide you safely if changes are needed.
What happens during weekly reviews
Each week, our team of experienced diabetes nurses and pharmacists securely reviews your CGM data.
If your readings are stable and within target, no action is needed. If we see patterns that could be improved, we may:
- Send you a message through the CareSens Air app
- Suggest small insulin dose adjustments
- Arrange a call (if needed)
All advice is personalised and focused on keeping you safe, steady and confident.

Do I need to contact you every week?
No, we’ll review your data and get in touch if advice or changes are needed.
Will someone always contact me after a review?
Not necessarily. If you don’t hear from us, it usually means your readings are stable and no changes are required.
What if I feel unwell or have very high or low readings?
Your CGM readings are reviewed weekly so if you feel unwell or need urgent advice, follow your usual guidance and contact your GP, NHS 111 or emergency services if needed.

Who provides this service?
This service is delivered by Spirit Health in partnership with your GP practice. Our clinical team includes experienced diabetes nurses and pharmacists who regularly support people using insulin and CGM. Spirit Health have been working with the NHS to deliver diabetes services for over 15 years.
Who can see my data?
Only our clinical diabetes team reviews your CGM data securely.
