We’re here to support you with more than just your CGM, CareSens Air – we’re here to help you take control of your diabetes with confidence, clarity, and care.

If you’ve been offered this service by your GP, you now have access to a dedicated team of diabetes specialists who will support you remotely. We work with you to better understand your glucose patterns, provide structured guidance, and adjust treatment when needed, safely and with your wellbeing at the heart of everything we do.

Information and Support

Your CGM Device

Your CareSens Air CGM (Continuous Glucose Monitor) gives you real-time insights into your glucose levels, so you can see what’s happening day and night without regular finger prick testing.
We’ll make sure you’re fully supported with everything you need to use your CGM effectively:

  • You’ll receive sensors and supplies directly from your GP via prescription.
  • We’ll help you understand how to apply, activate, and calibrate your CGM.
  • A back-up blood glucose meter is included for safety.

 

Join an online session led by our nurses and learn how to use your new device with live demos and a Q&A. These sessions are friendly, simple, and can be done from the comfort of home.

Your Digital Health Companion

Managing diabetes means more than collecting numbers. That’s why we’ve partnered with EMPOWER to give you access to easy-to-follow education resources in the Clinitouch app. This will help you to:

  • Interpret your CGM data and learn what that means
  • Spot trends and patterns
  • Know when to act (and when not to worry)

 

To get started:

  1. Scan the QR code below to download the app, or search for ‘Clinitouch Connect’ in the app store
  2. Login using your phone number
  3. We’ll send you a 6-digit code – enter this and you’ll be in!

Frequently Asked Questions

What is the CGM Support Service?

The CGM Support Service is a free, GP-referred programme designed to help you manage your diabetes with real-time glucose monitoring, clinical support, and education. Our team of diabetes specialists works with you remotely to adjust insulin safely and improve your glucose control.

How long is the service for?

The service typically lasts up to 3 months. During this time, we help you get the most out of your CGM device and optimise your diabetes management. Most patients choose to stay for the full duration to gain the greatest benefit.

What happens if my glucose readings are too high or too low?

If your readings are too low (hypoglycaemia), we may reduce your insulin. If they’re often high, we may recommend small increases. Any changes are made step-by-step every 3–5 days and only with your full understanding and agreement.

How is my data used?

Your CareSens Air CGM provides continuous insights into your glucose levels. We review this data weekly to help you stay within your target range and avoid highs and lows. Your data helps us offer safe, personalised guidance.

How are my GP records updated?

Our clinical team regularly reports your progress and any insulin changes to your GP. We update your patient record directly on the GP system, ensuring your GP stays informed at all times.

What if I need to be referred back to my GP?

We’ll refer you back to your GP at any time if needed, whether for further investigations, medication changes, or other clinical concerns. You’re always supported within the wider NHS care team.

Can I leave the service?

Yes. You can leave the service at any time—just let us know. However, most patients choose to stay for the full 3 months to get the most out of the support and education provided.

How do I connect with the clinical team?

You can connect with your healthcare team through the CareSens Air app using the code 329912.

This links your data directly to your team so we can monitor and support you more effectively.

Why do I need to download Clinitouch?

Clinitouch is the simple app where you can quickly, easily and securely access your diabetes education. Downloading the app is the only place to access this content, and soon we’ll be using Clinitouch to ask a few questions about how you’re feeling and your symptoms.

I can't login to Clinitouch?

Please choose ‘Log in with phone number’, click ‘Continue’ on the pop-up and then enter your mobile number.

Once you click continue, you should receive a text message from Clinitouch with your unique 6-digit code. Enter the code and click Log in.

Didn’t receive a code? Please check that you have entered your phone correctly, you can click ‘edit’ if you need to make any changes. You can click resend to generate a new code and try again.
